NHS New Out of Hours Arrangement from 1.10.2018

This new integrated out-of-hours service, starting at 11 AM on 1 October 2018, will be managed by the LAS (London Ambulance Service). ALL PATIENTS REQUIRING ACCESS TO THE OUT OF HOURS SERVICES WILL NEED TO RING 111. The old number, 020 8185 0545 is no longer in use. In effect, 111 will be the single point of telephone entry into urgent care services in the out of hours period (when the surgery is closed).

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of  your Tenancy agreement and a recent utility bill which needs to be dated within 3 months. If you do not have a Tenancy Agreement a signed letter from your Landlord will be acceptable. We also require one form of photographic ID, this is used for Online Services.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

You must bring all relevant information to the surgery for the receptionists to verify before you can be given an appointment to register. Please bring your information between 10:30am - 4:00 pm Mondays – Fridays.

Medical treatment is available from the date of registration. Please contact reception for further information.
