If you live within our practice area (see map at the bottom of the page) you are very welcome to register with us and our reception staff will be happy to guide you through the procedure. We may ask to see proof of ID and/or address in order to ensure we register you with the correct details. We also require one form of photographic ID, this is used for Online Services. There can be a very small number of instances where we may decline your request for registration such as your home address not being in our core area. This reason will be explained to you and we assure you that it will not be related to race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Once you have completed the online form, please visit the Practice between 10.30 AM - 4 PM Monday to Friday for verification of address and you will be given an appointment for registration.
